- When evaluating the accuracy of iron status tests, consider that serum ferritin is an acute-phase reactant21:
- A low ferritin level (<100 ng/mL in non-dialysis CRI patients) is a reliable indicator of iron deficiency
- A normal to moderately high serum ferritin level does not rule out iron deficiency or indicate adequate or excessive iron levels
- Iron parameters and Hgb should also be considered in combination to evaluate the cause of anemia before initiating any anemia treatment—including erythropoiesis stimulating agents (ESAs)7,20
